The Results: A Glimmer of Hope
The findings showed that DPP-4 inhibitors were associated with a reduced risk of dementia compared to sulfonylureas. Specifically, there were 4.4 dementia events per 1000 person-years for DPP-4 inhibitors versus 5.7 for sulfonylureas. The hazard ratio (HR) was 0.77 (95% CI, 0.71–0.85), indicating a significant reduction in risk. What's more, the longer patients used DPP-4 inhibitors and the higher the dose, the stronger the association with reduced dementia risk became. Similar observations were made across different dementia subtypes.
For GLP-1 RAs, the results were similar, although with a bit more uncertainty. Out of 181,215 initiators of GLP-1 RAs or sulfonylureas, followed for 530,415 person-years, GLP-1 RAs were associated with a reduced risk of dementia compared with sulfonylureas (2.3 versus 3.1 events per 1000 person-years; HR, 0.74; 95% CI, 0.46–1.18). The magnitude of the association increased with cumulative duration of use and dose.
